29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:   A) Mohican Structures, LLC on or about August 30, 2019 conducting work at 1100 Sugarbush Dr. Ashland, OH: The employer did not ensure a platform attachment was securely attached to the forks of the JLG rough terrain forklift. An employee performing wood framing activities from the platform in this condition was exposed to a potential fall of 29 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  A) Mohican Structures, LLC on or about August 30, 2019 at the address of 1100 Sugarbush Dr. Ashland, OH: The employer did not ensure employees were protected from falls while working from a job made scaffold platform.  The employee was exposed to a 29ft fall to the level below while installing fascia board and bringing up materials to the roof of the residential structure.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Mohican Structures, LLC, on or about August 30, 2019 at the address of 1100 Sugarbush Dr. Ashland, OH: The employer did not ensure employees were protected from falls while performing roofing activities for a residential type structure.  Employees were exposed to up to approximately 29 ft. falls to the ground below.
