29 CFR 1910.22(a)(2):  The employer must ensure the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition.  When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided:    At a workplace located at 5710 Vits Drive, Manitowoc, WI 54220; the employer did not ensure the floors of the pits for compactors 101, 102  and 103 were in a clean and dry condition. Hydraulic fluid (oil) leaking from the compactor equipment were causing puddles on walking surfaces of the pits, which created slick floors and stairs with the potentials for slips/trips.

29 CFR 1904.29(b)(1):  You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year:    At a facility located at 5710 Vits Drive, Manitowoc, WI 54220; the employer did not describe objects/substances that directly injured or made persons sick in column (F) of the OSHA Form 300 for 2019.    a)  An injury that occurred on May 6, 2019 was recorded with a description in column (F) that described the incident that lead to an injury. However, the record did not describe what the injury was.    b)  An injury that occurred on July 3, 2019 was recorded with a description in column (F) that described the injury that occurred, fractured thumb. However, the record did not describe the object/substances that directly injured the employee.
