29 CFR 1910.22(a)(3): Surface conditions. The employer must ensure: Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. On April 19, 2019, on the Roof and Breakroom Area of the Steam Generator Building, elevation 217 ft. Dynegy Zimmer Power Station, Enerfab employees located on the roof and breakroom area were exposed to a fire and explosion hazard from a hydrogen gas vent line that was venting hydrogen gas during the de-gassing of the steam generator turbine.

29 CFR 1910.1200(h)(1): Employee information and training. Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets. On April 15, 2019, Enerfab employees working at Dynegy Zimmer Power Station, 1781 U.S. 52, Moscow, were not provided with effective information and training on the hazards of hydrogen gas when the steam generator turbine was being de-gassed of hydrogen, and the precautionary measures that needed to be taken to protect their employees during the planned outage.
