OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a hazard of falling off the raised forks of a forklift: a) Warehouse area; on or about August 2, 2019 and at times prior thereto, the employer exposed employees to fall hazards in that employees were lifted into the air on the forks of a forklift, exposing the employees to a fall hazard of approximately 7 to 8 feet.

29 CFR 1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading: a) Loading Dock Area; On or about September 4, 2019 and at times prior thereto the employer exposed employees to fall hazards associated with a tractor trailer pulling away from the loading dock while the forklift is still entering the trailer to load and unload materials. The employer failed to ensure that wheel blocks were set in place to prevent movement of the trailers. GL Pack, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(m)(7), which was contained in OSHA inspection number 1365092, citation number 1, item number 2 and was affirmed as a final order on March 26, 2019, with respect to a workplace located at 309 Williamson Avenue, Opelika AL ,36801.
