29 CFR 1926.20(b)(4): The employer did not ensure that the employee(s) required to operate equipment and machinery was qualified by training or experience.    a) jobsite, Ellwood City Forge, 441 E. Main St., Corry, PA.  On or about September 3, 2019, and times prior thereto, employees accessed and entered the dumpster to disburse and rearrange debris within the dumpster.  On September 3, 2019, an employee suffered serious injuries when he fell out of the dumpster as he was disbursing the debris inside of it.     ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED:  Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
