The employer did not ensure that the safety monitor did not have other responsibilities which could take the monitor's attention from the monitoring function; penalty remains at $6,164.00. On or about March 02, 2022, during the repair of a roof for a commercial building located at 27000 Richmond Rd, Solon, OH 44139; the employer did not ensure employees working from a low-sloped roof were protected from a fall of approximately 17.2 feet by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.
