29 CFR 1910.67(c)(2)(v): A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift: On August 28, 2019, and at times prior, thereto, the employer did not ensure that employees working in aerial lifts were restrained within the aerial lift buckets to prevent ejection, exposing employees to a fall hazard of approximately 15 feet.
