29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Cell 6 Crimper, Equipment Number CR 09: On September 5, 2019 and at times prior, the employer exposed employees to potential hazardous energy sources such as, but not limited to, electric and hydraulic, in that the written procedures did not include specific steps for isolating, securing and verifying that electric and hydraulic energy was released. The procedures did not describe how to verify that the hydraulic pressure has been released. The procedures also failed to notify affected employees and state how to reverse the lockout procedures. b) Cell 6 Crimper, Equipment Number CR05: On September 5, 2019 and at times prior, the employer exposed employees to potential hazardous energy sources such as, but not limited to, electric and hydraulic, in that the written procedures did not include specific steps for isolating, securing and verifying that electric and hydraulic energy was released. The procedures did not describe how to verify that the hydraulic pressure has been released. The procedures also failed to notify affected employees and state how to reverse the lockout procedures. c) Fin Mill 12, Equipment Number FM12: On September 5, 2019 and at times prior, the employer exposed employees to potential hazardous energy sources such as, but not limited to, electric and pneumatic, in that the written procedures did not include specific steps for isolating, securing and verifying that electric and pneumatic energy was released. The procedures did not describe how or where to lock the disconnect or how to verify that the pneumatic pressure has been released. The procedures also failed to notify affected employees and state how to reverse the lockout procedures.
