29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   On or about September 5, 2019, at the south end of a parking garage under restoration located at 2841 N. Ocean Boulevard in Fort Lauderdale, Florida, employees were exposed to a fall hazard of approximately 5 feet - 9 inches when performing concrete restoration work from the work level platform of a fabricated frame scaffold that was not fully planked.

Ocean Boulevard in Fort Lauderdale, Florida, employees were exposed to hazards associated with performing concrete restoration work prior to the employer providing an inspection of the worksite by a competent person to recognize potential hazards and determine the proper materials and equipment needed to perform the work safely.

Ocean Boulevard in Fort Lauderdale, Florida, employees were exposed to fall hazards of approximately 5 feet - 9 inches when performing concrete restoration work from a fabricated frame scaffold that was not erected under the direction of and/or by a competent person in scaffold erection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about September 5, 2019, at the northwest corner of a parking garage under restoration located at 2841 N. Ocean Boulevard in Fort Lauderdale, Florida, an employee was exposed to a fall hazard of approximately 9 feet - 9 inches when performing concrete restoration work from a walking/working surface while not having the legs straps of his safety harness engaged and secured to prevent him from sliding out of the harness in the event that he fell.

Ocean Boulevard in Fort Lauderdale, Florida, employees were exposed to a fall hazard of approximately 9 feet - 9 inches when performing concrete restoration work from a walking/working surface while having the snap hooks of their shock absorbing lanyards engaged directly to a natural fiber rope horizontal lifeline.
