29 CFR 1910.184(c)(6): Safe Operating Practices: Slings shall be securely attached to their loads. On September 4, 2019, at Press #10, after moving a 3,000 lb. steel coil using a 5 ton crane and single-leg alloy steel chain sling to a position next to the press feeder cradle, the sling was not securely attached to the steel coil before the coil was secured from falling and lifted into the cradle of the press feeder cradle.

29 CFR 1910.184(c)(14): Employers must not use slings without affixed and legible identification markings. On September 4, 2019, at Press#10, an employee had used a single leg alloy steel chain with a metal tag that was illegible identification markings to lift and move an approximately 3,000 lb. steel coil over to where it would be lifted into a press feeder cradle.

29 CFR 1910.184(e)(3)(i): Inspections; In addition to the inspection required by paragraph (d) of this section, a thorough periodic inspection of alloy steel chain slings in use shall be made on a regular basis, to be determined on the basis of (A) frequency of sling use; (B) severity of service conditions; (C) nature of lifts being made; and (D) experience gained on the service life of slings used in similar circumstances. Such inspections shall in no event be at intervals greater than once every 12 months. The employer had not completed a thorough periodic inspection of alloy steel chain slings in use as the single-leg alloy steel chain sling used on September 4, 2019, at Press #10 in intervals no greater than once every 12 months.
