29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling: a) foam deadner mixer platform; on September 4, 2019 the employer exposed employees to falls of about 117 inches to the floor below by not ensuring that the loading gate was protected for falls. The opening has a gate but it can't be closed due to the materials blocking the path. The material is not large enough to completely guard the opening.
