29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Production Areas (PAD Warehouse Department): On or about June 12, 2023, and at times prior, the employer exposed employees to amputation hazards, in that an energy control program (Lock Out/Tag Out-LOTO) training was not provided to employees performing service and maintenance activities such as, but not limited to "end of shift hand cleaning" of the Extruder #2, Extruder #3 and Extruder #4 machines.
