29 CFR 1910.147(c)(4)(i) Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:    a)  On or about September 6, 2019, the employer did not develop, document and utilize procedures for the control of hazardous energy such as but not limited to electrical for employees conducting setup activates on the Slitter Machine of the Fury Slitter Line.

29 CFR 1910.147(c)(7)(i)(A): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:     a)  On or about September 6, 2019, the employer did not ensure that employees were trained as authorized employees in the proper lockout tag-out procedures for isolating the Slitter Machine on the Fury Slitter Line.

29 CFR 1910.147(d) Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    a) On or about September 6, 2019, Employees who performed servicing and maintenance on and around the Slitter machine on the Fury Slitter Line. were not required to:     (1) know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy,   (2) Turn off and shut down the machines,   (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,   (4) affix lockout or tagout devices to each energy isolating device,   (5) prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.

29 CFR 1910.212(a)(1)  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    b) On or about September 6, 2019, employees working on and around the Sheer machine on the Thor slitter line were exposed to a point of operation hazards from the cutting blade of the sheer that was not guarded.     c) On or about September 6, 2019, employees working on and around the Pinch Rollers on the Thor slitter line were exposed to unguarded ingoing nip points hazards create by the Pinch Rollers rotating inward toward the Slitter.    d) On or about September 6, 2019, employees working on and around the Slitter on the Thor slitter line were exposed to a point of operation hazards create by the rotating slitter blades routing inward toward the re-coiler.     e) On or about September 6, 2019, employees working on and around the Re-coiler on the Thor slitter line were exposed to unguarded ingoing nip points hazards create by metal being re-coiled on itself.    f) On or about September 6, 2019, employees working on and around the Scrap roller on the Thor slitter line were exposed to unguarded ingoing nip points hazards create by metal being re-coiled on itself.     h) On or about September 6, 2019, employees working on and around the Slitter on the Fury slitter line were exposed to a point of operation hazards create by the rotating slitter blades routing inward toward the re-coiler.     I) On or about September 6, 2019, employees working on and around the Re-coiler on the Fury slitter line were exposed to unguarded ingoing nip points hazards create by metal being re-coiled on itself.    j) On or about September 6, 2019, employees working on and around the Scrap roller on the Fury slitter line were exposed to unguarded ingoing nip points hazards create by metal being re-coiled on itself.

29 CFR 1910.219(c)(4)(i)  Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves:    c) On or about September 6, 2019, employees working on and around the Un-coiler machine on the Fury slitter line were exposed to an unprotected rotating shaft. The projecting unguarded shaft exceeded the permitted length.    d) On or about September 6, 2019, employees working on and around the Re-coiler machine on the Fury slitter line were exposed to an unprotected rotating shaft  The projecting unguarded shaft exceeded the permitted length.
