29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about March 26, 2026, the employer did not ensure that the automatic coil pusher on the Thor/Fury coil turnstile was guarded to prevent employee exposure to the point of operation while transferring coils onto the Braner down layer table. Employees were exposed to a caught between hazard created by the unguarded pusher advancing toward the coil  b) On or about March 26, 2026, the employer did not ensure that the retractable horn on the Braner Coil Down Layer Table was guarded to prevent employee exposure to pinch points while employees were transferring coils from the Thor/Fury coil turnstile onto the table. Employees were exposed to a pinch point hazard created by the retractable horn moving toward the coil turnstile arms.  c) On or about March 26, 2026, the employer did not ensure that the lifting and descending Braner Coil Down Layer Table was guarded to prevent employee exposure to pinch points while employees were transferring coils from a coil turnstile onto the table. Employees were exposed to a pinch point hazard created by the table moving toward its supporting structure.

29 CFR  1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  a) On or about March 26th, 2026, the employer did not ensure their procedures for the control of hazardous energy for the Fury Thor Banding line down-layer table contained specific procedural steps. Specifically, the procedure did not include complete steps to isolate, relieve, and restrain stored or potential energy, for the down layer table's hydraulic/gravitational energy such as lowering the down layer table or blocking if serviced in the raised position thereby exposing maintenance employees to struck-by hazards and crushing hazards.

29 CFR  1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  a) On or about April 1st, 2026, the employer did not properly certify the periodic inspection of the energy control procedure for the Fury Thor Banding Line. The certification record did not identify the employees included in the inspection. As a result, the employer failed to document all mandatory elements of the annual inspection certification.
