Cross braces were used as a means of access. Location - 135 South St. Chicopee, MA On or about 09/11/2019, employees were exposed to falls greater than 15 Ft. to a lower level while utilizing an ladder jack scaffold pick greater than 2 Ft. below the upper landing for access and egress.
