29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  a)  Alejandros Roofing, LLC, 4672 W 20th Street, Greeley, CO  80634:  On and before September 12, 2019, the employer did not ensure that each employee engaged in reroofing activities 6 feetor more above lower levels on residential roofs was protected by personal fall arrest system.   Employees were tearing off shingles on a two-story residential building without the use of fall protection. This condition exposed employees to fall hazards of up to 20 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that did not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:  a)  Alejandros Roofing, LLC, 4672 W 20th Street, Greeley, CO  80634:  On and before September 12, 2019, the employer did not ensure that when portable ladders were used for access to residential roofs, the ladder side rails were extended at least 3 feet above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that did not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder. 29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, or a barricade was not used to keep the activities or traffic away from the ladder:  a)  Alejandros Roofing, LLC, 4672 W 20th Street, Greeley, CO  80634:  On and before September 12, 2019, the employer did not ensure that ladders placed in front of doorways and garages could not be displaced by workplace activities or traffic.  The employer did not ensure that the ladders were secured to prevent accidental displacement, or that barricades were used to keep the activities or traffic away from the ladders.  This condition exposed employees to a fall hazard of up to 10 feet.
