On or about September 13, 2019, employees operating the Makita Circular Saw, s/n 1864327, were exposed to an amputation injury in that the blade guard had been defeated by shimming the sliding guard to the open position.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    a.    On or about September 13, 2019, employees were exposed to a 120-volt electric shock hazard in that the power cord on the Makita Circular Saw, s/n 1864327, had been damaged and field repaired in two locations, exposing live conductive wires that were not covered with adequate insulative material.    b.    On or about September 13, 2019, employees were exposed to a 120-volt electric shock hazard from an extension cord connecting the Makita Circular Saw, s/n 1864327, that had been damaged and field repaired, exposing live conductive wires that were not covered with adequate insulative material.    c.    On or about September 13, 2019, employees were exposed to a 120-volt electric shock hazard in that the power cord on the Makita Circular Saw, s/n 2107609, had been damaged and field repaired, exposing live conductive wires that were not covered with adequate insulative material.    d.    On or about September 13, 2019, employees were exposed to a 120-volt electric shock hazard in that the power cord on the Makita Circular Saw, s/n 2107910, had been damaged and field repaired in two locations, exposing live conductive wires that were not covered with adequate insulative material.    e.    On or about September 13, 2019, employees were exposed to a 120-volt electric shock hazard in that plug of the extension cord connecting the Bosch Bulldog Xtreme drill was shattered, exposing live conductors

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    a.    On or about September 13, 2019, at the peak of the roof trusses, a framing employee installing hip bracing framing was exposed to a 19 feet fall hazard in that he was not protected by a fall protection system.    b.    On or about September 13, 2019, over the front entrance, a framing employee installing wood frames was exposed to a 10 feet fall hazard in that he was not protected by a fall protection system.

On or about September 13, 2019, in the garage, a framing employee was exposed to a 6 feet fall hazard in that he was standing on the second to top step and backside ladder brace of a 6-foot stepladder.  b.    On or about September 13, 2019, at the northwest corner of the house, a framing employee was exposed to a fall hazard in that he was standing on the top and top step of a 6-foot stepladder.
