29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws: a) Maintenance area, inside captive barge, 5913 Upper River Road, Harrods Creek, KY: On or about 11 Sep 2019, the electrical cord on the bandsaw going into the motor and into the on/off power switch was pulled free from the coupling exposing exposing workers to electrical hazards. b) On the captive barge, 5913 Upper River Road, Harrods Creek, KY: On or about 11 Sep 2019, an electrical extension cord powering a light on the corner of the barge was pulled loose from the female end of the cord exposing workers to electric shock. c) On the captive barge, 5913 Upper River Road, Harrods Creek, KY: On or about 11 Sep 2019, an electrical extension cord powering equipment on the barge was pulled loose from the female end of the cord exposing workers to electric shock.
