29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards:  a)  On September 9, 2019, employees were permitted to work in and near the traveled way of Green Bay Road and Tamble Avenue without providing signage indicating how motor vehicle traffic can move through the temporary traffic control zone as required by MUTCD Section 6G.03.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) On September 9, 2019, an employee conducting water line investigation in a trench, six (6) feet and five (5) inch in depth, was not provided with means of egress.   No abatement certification or documentation is required for this item.    Mango Construction, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.651(c)(2), which was contained in OSHA inspection number 1069327, citation number 1, item number 1 and was affirmed as a final order on October 14, 2016, with respect to a workplace located at Eagle Street, Parkview Mobile Home Park, in Hodgkins, Illinois.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a) On September 9, 2019, an employee was exposed to cave-in hazards while performing work in an unprotected trench of six (6) feet five (5) inches deep. The employer did not ensure that the employee was protected from cave-in hazards.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
