29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: On or about September 16, 2019 at the worksite located at 4479 Big Rock Trail (Lot 101) within the Mundy Mill Subdivision in the city of Oakwood, Georgia, employees were exposed to electrical shock and/or burns, when the employer failed to remove from service damaged flexible cords including but not limited to the following: a) A yellow extension cord providing power from the temporary power pole to a pig tail, that was used to power other pieces of equipment, had broken outer jacket, had broken female receptacle end . b) A red extension cord that was used to power a Makita 5007F circular saw S/N #193127 had broken outer jacket. c) A red extension cord that was used to power a Makita 5007F circular saw S/N #1789823 had exposed copper wires. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:(a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY) On or about September 16, 2019 at the worksite located at 4479 Big Rock Trail (Lot 101) within the Mundy Mill Subdivision in the city of Oakwood, Georgia, employees were exposed to electrical shock and/or burns, when the employer failed to remove from service damaged flexible cords including but not limited to the following: a) A yellow extension cord providing power from the temporary power pole to a pig tail, that was used to power other pieces of equipment, , had broken female receptacle end at the grounding portion of the receptacle, causing the ground not to be continuous. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
