29 CFR 1904.39(a)(2): The in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident was not reported within 24 hours. a) On or about 7/21/2019: An employee was hospitalized for 2nd and 3rd degree burns on 7/21/2019 and the employer's report of the incident was not submitted to OSHA until 09/19/2019. b) On or about 9/10/2019: An employee was hospitalized for a puncture injury on 9/10/2019 and the employer's report of the incident was not submitted to OSHA until 9/12/2019.
