29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about September 17, 2019, an employee, operating the stay machine, was exposed to hazards associated with moving parts during machine operation and a suitable guard was not in place to protect the employee from the hydraulic powered RAM.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a. On or about September 17, 2019, an employee , operating the stay machine, was exposed to electrical hazards due to the lack of strain relief on the flexible cord entering an electrical box because the insulation was pulled away from the plug. b. On or about September 17, 2019, an employee, operating the stay machine, was exposed to electrical hazards due to the lack of strain relief on the cord supplying power to a limit switch because the insulation was pulled away from the plug.
