Warehouse, Receiving Dock Door 4: On or about September 20, 2019, and at times prior, the employer exposed employees to electrical hazards, in that an extension cord used to provide power to an industrial fan was attached with plastic cable ties to the metal framework of an overhead storage rack system. b. Warehouse, Receiving Dock Door 10: On or about September 20, 2019, and at times prior, the employer exposed employees to electrical hazards, in that an extension cord used to provide power to an industrial fan was attached with plastic cable ties to the metal framework of an overhead storage rack system. c.
