DBA Premium Panels, Arvada CO: On or about September 18, and at times prior, employees were exposed to caught in, crushing, and amputation hazards due to lack of point of operation guarding on the Schectl 310 Automatic Press Brake (S/N T40397). Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
