29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Drexel Metal Inc. DBA Premium Panels, Arvada CO: On or before 8/19/2020 the employer did not protect employees from injury in that the Roll Former Corp Stand Alone Perf [SN# RF6924] was lacking adequate guarding. Roll former wheels were exposed and not guarded in accordance with this standard. This condition exposed employees to amputation and crushing hazards.

Arvada CO: On or before 8/19/2020, the employer did not protect employees from injury in that employees were exposed to caught in, crushing, and amputation hazards due to lack of point of operation guarding on the RAS Reinhardt Machinenbau RAS 62.30 (folder 3). b) Drexel Metals Inc. Arvada CO: On or before 8/19/2020, the employer did not protect employees from injury in that employees were exposed to caught in, crushing, and amputation hazards due to lack of point of operation guarding on the Schectl 310 Automatic Press Brake (S/N T40397)(folder 2). Drexel Metals Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(iv)(d), which was contained in OSHA inspection number 1432751, citation number 1, item number 1, and was affirmed as a final order on 12/19/2019, with respect to a workplace located at 6270 Joyce Dr. Arvada, CO 80403.
