The gates provided in the guardrail system where left open when order picker trucks were not parked in the bays, and; B. Employees operating order picker trucks were disengaging from personal fall arrest equipment to egress and access order picker trucks, which exposed them to fall hazards from edge of the mezzanine to the warehouse floor below.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: On, or about, September 23, 2019, in the following locations, material was stored within exit routes, including areas of refuge: A. Warehouse: Pallets of material were stored in front of an exit door that led from Aisle W to another section of the warehouse. B. Mezzanine: The access to the front exit stairwell was blocked by a pallet of boxes. Once entry was gained to the stairwell, the top landing was blocked by a box and a pallet, the lower flight of stairs had sections of gutters on the stairs and the lower landing was blocked by a generator, electrical panel and air compressor. C. Exit Stairwell located on Side of Building: The lower landing was used for storage, including paint, oil, ice melter and containers of gasoline.
