29 CFR 1910.140(d)(1)(ii): The employer did not ensure that fall protection systems bring employees to a complete stop and limit the maximum deceleration distance the employee travels to 3.5 feet (1.1m): Warehouse: On, or about, June 13, 2022, an employee who selected orders from elevated storage rack bays was provided with a Miller Scorpion Personal Fall Limiter (PFL) that was rated for a capacity of 310 pounds. The weight limit of the device was not sufficient for the employee's stated weight which could have caused a failure of the braking system in the PFL and allowed the lanyard webbing to deploy the entire nine foot length in the event of a fall.

The leg straps on the harness were not fastened in place, which could have caused the chest strap to contact his neck/chin in the event of a fall.
