On or about September 24, 2019, at the jobsite, the tubular welded scaffolding used by employees had one plank installed and it extended three inches past the support brace.

The scaffolding was missing cross bracing, the working levels of the scaffolding were not fully planked, and guardrails were not installed on the end section of one the scaffolds. Employees were exposed to fall hazards up to 16 feet 4 inches.

On or about September 24, 2019, at the jobsite, a competent person did not inspect the tubular welded scaffolding prior to employees working from the scaffolding. The scaffolding was missing cross bracing, the working levels of the scaffolding were not fully planked, and guardrails were not installed on the end section of one the scaffolds. Employees were exposed to fall hazards up to 16 feet 4 inches.

On or about September 24, 2019, at the jobsite, untrained employees erected, worked on and dismantled tubular welded scaffolding without the being trained on the proper methods of erecting scaffolding and the fall hazards of working from scaffolding. The scaffolding was missing cross bracing, the working levels of the scaffolding were not fully planked, and guardrails were not installed on the end section of one the scaffolds. Employees were exposed to fall hazards up to 16 feet 4 inches.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:  a.  On or about September 24, 2019, at the jobsite, employees were building a concrete block wall on the second floor of a new residential property and the area just above the garage was not guarded.  An employee was observed standing directly at the unprotected edge, thus exposing the employee to a 11 foot 4 inch fall hazard.

On or about September 24, 2019, at the jobsite, employees were building a concrete block wall on the second floor of a new residential property and the area just above the garage had unprotected rebar extending upward, thus exposing employees to impalement hazards.
