29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  Site: On the portico at 39 Fairview Avenue, Chicopee, MA 01013: On 10/1/2019 a portable ladder used to access the top of the portico was not positioned so that the ladder side rails extended 3 feet above the access point on the portico, requiring employees to steps laterally from the ladder to access the portico's landing surface.
