29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:  a) 3450 Main Road East, Emmaus, PA, Worksite, Roof - on or about October 1, 2019, employees were performing residential roofing work while using a ladder jack scaffold that had its platform ends extended more than 18 inches beyond its supports.

29 CFR 1926.452(k)(2): The support used for ladder jack scaffolds did not meet the requirements of Subpart X - Stairways and Ladders:  a) 3450 Main Road East, Emmaus, PA, Worksite, Roof - on or about October 1, 2019, employees were performing residential roofing work while using a ladder jack scaffold that was supported by a pair of extension ladders that were used at an angle such that the horizontal distance from the top support to the foot of the ladder is in excess of one-quarter of the working length of the ladder.  Abatement Note:  To prevent a ladder from being set too shallow, follow the ladder positioning guidance in accordance with 29 CFR 1926.1053(b)(5)(i).
