29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided. On the east side of the construction site, the employer did not provide an extension ladder where employees could access and egress the residential home upper landings. The employees were exposed to potential fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On the east side of the construction site, employees engaged in decking work without fall protection, were exposed to fall hazards approximately 9 feet to the lower level. Jose Valdez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1328128, citation number 01, item number 001 and was affirmed as a final order on 08/07/2018, with respect to a workplace located at 2154 Slowstream Dr., Royse City, Texas 75189. Jose Valdez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1275215, citation number 01, item number 003 and was affirmed as a final order on 12/19/2017, with respect to a workplace located at 3509 Valmur Ave., Colleyville, Texas 76034.
