29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Future Site of Woodland Prep Charter School (North Foundation Block): On or about August 06, 2019, the employer failed to instruct or train their employees in recognizing, eliminating or controlling hazards associated with rigger activities using a Komatsu Hydraulic Excavator which exposed their employees to hazards such as but not limited to caught-in-between hazards, struck-by hazards and crushed-by hazards during construction operations.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Future Site of Woodland Prep Charter School (North Foundation Block): On or about August 06, 2019, an employee suffered a workplace injury that resulted in an in-patient hospitalization for treatment and care. The employer failed to report the work-related hospitalization to OSHA within twenty-four (24) hours.
