29 CFR 1910.305(b)(1)(ii) as required by 1960.8(b): Unused openings in boxes, cabinets, or fittings were not effectively closed:   Machine Room:    a) Electrical panel labeled CP PNL had an unused circuit breaker opening that was not closed.  Employees were exposed to electrical and arc flash hazards.  b) The high voltage electrical panel had two electrical bucket switches removed and covered with cardboard and tape instead of a manufacture approved cover.  Employees were exposed to electrical and arc flash hazards.  The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 1232772, Notice Number 1, Item Number 1, and was affirmed as a final order on 08/08/2017, with respect to a workplace located at 17601 25th Street Beale AFB, CA 95903.  The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 1097414, Notice Number 1, Item Number 2, and was affirmed as a final order on 11/13/2015, with respect to a workplace located at Building 601 1st Box 31344 Fort Greely, AK 99731.
