29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about October 4, 2019, an employee was observed working without a hard hat on the ground level while an employee with a pneumatic nail gun was installing felts and shingles above him. This condition exposed the employee to struck by hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about October 4, 2019, an employee installing shingles at 9224 Wetherbee Circle, Sherwood, AR was operating a pneumatic nail gun with no eye and/or face protection. This condition exposed the employee to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 4, 2019, employees installing shingles on the rooftop at 9224 Wetherbee Circle, Sherwood, AR wore full body harnesses that were not securely buckled, connected to lanyards, and/or anchor points. Employees were exposed to fall from height hazards of approximately 14 feet above ground level on the west facing eave.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about October 4, 2019, at 9224 Wetherbee Circle, employees exposed to fall from height while working on a residential rooftop were not provided training to recognize and minimize the hazards associated with falls.
