29 CFR 1910.119(j)(6)(ii): Appropriate checks and inspections were not performed to assure that equipment was installed properly and consistent with design specifications and the manufacturer's instructions. At the worksite located at 2504 Industrial Drive, Kirksville, Missouri: The installation of the ammonia piping on the Granite side of the facility was not installed in accordance with the P&ID. The PHA checklist dated 1/25/16, recommended field inspection of piping during installation. The PHA noted inspections would continue throughout the installation process on 10/15/16. Field inspections did not continue in accordance with the PHA.
