On or about October 31, 2019, in the fabrication shop, employees were exposed to electrical hazards by utilizing an extension cord that was plugged into other extension cords, which prohibited in the listing and labeling.    b.  On or about October 31, 2019, employees in the fabrication shop were exposed to electrical hazards due to an extension cord being used in wet conditions, which was prohibited in the listing and labeling.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    On or about October 31, 2019, in the fabrication shop where employees were exposed to electrical hazards from flexible cords, located on the top of a concrete separation wall, which were used as a substitute for fixed wiring.
