29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: The controlling contractor is failing to ensure that all employees of the sub-contractor Clean & Secure Roofing LLC are protected from fall hazards associated with using ladders during residential construction. This was most recently documented on or about October 7, 2019 at 9223 Dorcas Street, Omaha, NE 68124. Four employees from Clean & Secure Roofing LLC utilized a ladder that was not extended 3 feet over the upper landing surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The controlling contractor is failing to ensure that all employees of the sub-contractor Clean & Secure Roofing LLC are protected from fall hazards while working on a residential roof. This was most recently documented on or about October 7, 2019 on the jobsite located at 9223 Dorcas Street, Omaha, NE 68124. Four employees from Clean & Secure Roofing LLC were observed removing old shingling on the roof of an existing house. The four employees were not utilizing any form of fall protection. McKinnis Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1121074, citation number 1, item number 2, and was affirmed as a final order on May 23, 2016 with respect to a workplace located at 17551 Pinkney Street, Omaha, NE 68516. McKinnis Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1214117, citation number 1, item number 1, and was affirmed as a final order on June 1, 2017 with respect to a workplace located at 2810 Copper Ridge Drive, Lincoln, NE 68516.
