29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:  a)     USDA Farm Service Agency at 528 91st Avenue NE Suite C Lake Stevens, WA--Back Exit Door;  The Amerex fire extinguisher located near the exit door was not visually inspected monthly.   b)     USDA Farm Service Agency at 528 91st Avenue NE Suite C Lake Stevens, WA--Server Room;  The Amerex fire extinguisher was not visually inspected monthly.   The USDA Farm Service Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1068423, citation number 1, item number 1 and was affirmed as a final order on September 28, 2015, with respect to a workplace located at Spokane Valley, Washington.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)  USDA Farm Service Agency at 528 91st Avenue NE Suite C Lake Stevens, WA--Lunchroom;  A Magic Chef microwave was plugged into a surge protector power strip (relocatable power tap).  The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.    The USDA Farm Service Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1068423, citation number 1, item number 1 and was affirmed as a final order on September 28, 2015, with respect to a workplace located at Spokane Valley, Washington.  The USDA Farm Service Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1046846, citation number 1, item number 1 and was affirmed as a final order on March 17, 2015, with respect to a workplace located at Brush Prairie, Washington.
