29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about October 2, 2019, an employee was hospitalized after being struck by a bolt that had fallen from approximately 200 feet above and the employer failed to report the hospitalization within the required 24 hours.
