29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:    (a)  York Reduction Systems, LP - Production Floor - The employer did not develop a noise monitoring program to determine employee noise exposure levels, on or about October 8, 2019.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:    (a)  York Reduction Systems, LP - Plantwide - The employer did not provide the information contained in Appendix D to the employees using single use half-mask Sperian One-Fit Model N95 respirators during tasks such as cleaning the filter press or maintenance activities, on or about October 8, 2019.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
