29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s):  (a)  PRODUCTION AREA - The Unicarriers FHD30T5V forklift was in need of repair, in that the parking brake was known to be inoperative, and the forklift was not taken out of service and had continued to be used, on or about October 8, 2019.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour:    (a)  COMPRESSED GAS CYLINDER STORAGE AREA - The employer stored three oxygen cylinders with two acetylene cylinders in the full cylinder storage area, and two oxygen cylinders with one acetylene cylinder in the empty cylinder storage area, on or about October 8, 2019.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
