29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower lever by one or more of the following: Fly Ash Hopper. On or about October 07, 2019, and at times prior thereto, the walkway surrounding the hopper had an open hole of approximately two feet wide by four feet long. Employees were exposed to a fall hazard of more than four feet.

Cement Weight Hopper. On or about October 07, 2019, and at times prior thereto, a midrail was not installed on the north and east side of the hopper walkway. Employees were exposed to a fall hazard of more than four feet.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): Chemical Storage Room - Compressor #1. On or about October 07, 2019, and at times prior thereto, motor shaft, pulleys, and belts of the air compressor units were not fully protected with fixed or movable guards to prevent contact with the rotating and moving parts. Employees were exposed to caught-in hazards from the rotating parts of the compressor.
