29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   a.)     On or about October 9, 2019, the employer did not ensure periodic audits of the energy control procedures were completed at least annually. Employees are required to perform mold change activities on machines including but not limited to, the #11 Reed 300T Plastic Injection Molding Machine (Serial #P990455), which require energy control procedures that have not been reviewed for deficiencies.

On or about October 9, 2019, the employer did not ensure that employees were adequately trained in the lockout procedures for the #11 Reed 300T Plastic Injection Molding Machine (Serial #P990455).  Employees did not lock out and tag out the electrical panel while performing mold change activities on equipment including, but not limited to the #11 Plastic Injection Molding Machine.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a.)     On or about October 9, 2019, the employer did not ensure employees were protected while performing mold changes on plastic injection molding machines, including but not limited to #11 Reed 300T Plastic Injection Molding machine (serial #990455). The employer did not ensure that the energy control application steps to control the electrical and hydraulic energy sources were implemented prior to the maintenance tasks, such as but not limited to mold changes, the employer did not:               �  Shut down the equipment using the procedures established. �  Physically operate energy isolating devices to control energy sources and affix lockout device(s). �  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. �  Verify that isolation and deenergization of the machine or equipment had been accomplished.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:   a.) On or about October 9, 2019, the employer did not ensure that the Toyota Electric sit down Model 7FBEU20, Serial #13729 forklift was properly marked to include the homemade, unapproved front-end attachment while showing the approximate weight of the lift truck and attachment combination at maximum elevation with the load laterally centered.  Employees were not aware of the lifting capacity of the lift truck and lifting device, leaving a potential for tip-over from being overloaded, exposing employees to crushing hazards.

Employees were exposed to struck by hazards while lifting molds in and out of the plastic injection molding machines.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:   a.)     On or about October 9, 2019, the employer did not ensure the employees were protected while using the 110V fan at the #11 300T Reed plastic injection molding machine (Serial #P990455). Employees must plug the fans in each time of use and are exposed to shock hazards due to exposed conduit near the plug.
