29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items A-D of this section:  FRC East, Bldg. Askar Shop #69140, Safe Site B-10:  On or about October 8,  2019, the energy control procedure, for the LSIS 22, failed to clearly identify all of the specific steps to be followed by employees to isolate the stacker lift cage and control the release of stored energy.  The procedure also lacked specific steps to block, clamp or restrain the lift cage along the mast of the machine prior to performing maintenance activity and specific verification steps to take in order to determine the effectiveness of the energy control measures.     ABATEMENT DOCUMENTATION REQUIRED

Askar Shop #69140, Safe Site B-10:  On or about October 8, 2019, the employer did not ensure the periodic inspection and implementation of energy control procedures in order to verify that the procedures being used by authorized employees are adequate and being properly applied. Askar Shop #69140, Safe Site B-10: On or about October 8, 2019, the employer did not ensure that the training being provided included sufficient detail and specificity to permit an authorized employee to safely and effectively utilize isolating devices such as, brackets and clamps, to control the energy that was unexpectedly released by the lift cage. 29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  FRC East, Bldg. Askar Shop #69140, Safe Site B-10: On or about October 8, 2019, the attempted servomotor replacement for the LSIS 22 resulting in the unintended release of storage energy from the overhead Lift Cage due to the established energy control procedures not including the specific energy isolating devices that were needed to control the hazardous energy.  Authorized employees were not made aware of the specific isolating devices to be used in order to isolate the equipment from the energy sources, therefore, LOTO devices could not be affixed in accordance with �1910.147(d)(4).  ABATEMENT DOCUMENTATION REQUIERD
