29 CFR 1926.352(d): Suitable fire extinguishing equipment was not immediately available in the work area(s) where welding, cutting, or heating was being performed:  At the worksite located at 453 South Patton Avenue, Springfield, Missouri 65806, employees were exposed to serious fire and burn hazards. Employees working on a low-sloped roof were conducting welding operations when installing thermoplastic polyolefin roofing. Employees were utilizing JM membrane bonding adhesive near the welding operations of the thermoplastic polyolefin without a fire extinguisher or other suitable means of fire protection on the roof.

29 CFR 1926.404(f)(3)(iii): A neutral conductor was not bonded to the generator frame where the generator was a component of a separately derived system:  At the worksite located at 453 South Patton Avenue, Springfield, Missouri 65806, employees were exposed to serious electrical shock and burn hazards. The WINCO WL12000HE portable generator did not have a neutral conductor bonded to the generator frame. The generator was a component of a separately derived system.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  At the worksite located at 453 South Patton Avenue, Springfield, Missouri 65806, employees were exposed to serious fall hazards. Employees working on a low-sloped roof were stepping over from one roof to another roof to access the water cooler. The distance between roofs was measured at 18 1/2 inches wide. Employees were exposed to a fall hazard of 12 feet to the ground below.
