29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder:   a) 458 Center Street, Bethlehem, PA, Worksite - on or about October 11, 2019, employees were performing roofing work while accessing the upper working area, approximately 23.5 feet above grade, via a Werner D1332-2 32 foot extension ladder that was set where the base horizontal distance was in excess of one-quarter of its working length.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 458 Center Street, Bethlehem, PA, Worksite - on or about October 11, 2019, employees were utilizing a Werner D1332-2 aluminum extension ladder to access the roof of a structure, approximately 23.5 feet above grade, that was not extended at least three feet above the upper landing surface.   b) 458 Center Street, Bethlehem, PA, Worksite - on or about October 11, 2019, employees were utilizing two Werner D1320-2 aluminum extension ladders to access the various roof working areas of a structure, approximately 12 to 23.5 feet above grade, that were not extended at least three feet above the upper landing surface.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a) 458 Center Street, Bethlehem, PA, Worksite - on or about October 11, 2019, a Werner D1220-2 20 foot aluminum extension ladder was in use to access the upper roof area of a structure, but the ladder was damaged in multiple locations along it's right side rails and on one its base section rungs.   Webb Contracting Corp. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.1053(b)(16) which was contained in OSHA inspection number 1421585, citation number 1, item number 3b and was affirmed as a final order on September 4, 2019, with respect to a workplace located at 357 Chilton Court, Plymouth Meeting, PA 19462.  Webb Contracting Corp. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.1053(b)(16) which was contained in OSHA inspection number 1382003, citation number 1, item number 3b and was affirmed as a final order on April 30, 2019, with respect to a workplace located at Twin Ponds Road, Breinigsville, PA 18031.
