The machine was missing guard and electrical wires were modified and taped with masking tape, thus exposing employees to amputation and electrical hazards.

Dyer Insulation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.1200 (h)(3)(ii), which was contained in OSHA inspection number 1186080, citation number 2, item number 1 and was affirmed as a final order on September 18, 2017, with respect to a workplace located at 2170 N. Central Ave. Fort Lee, New Jersey.    Dyer Insulation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.1200 (h)(3)(ii), which was contained in OSHA inspection number 1127837, citation number 1, item number 1B and was affirmed as a final order on August 25, 2016, with respect to a workplace located at 129 Littleton Ave, Newark, New Jersey.
