29 CFR 1926.452(w)(6)(i): Employee(s) were allowed to ride on scaffold(s) when the surface on which the scaffold was being moved was not within 3 degrees of level, and free of pits, holes, and obstructions:     Worksite:  The surface upon which the Sky Jack III scissor lift (scissors lift) was traveling with two (2) employees to get to a work location was uneven and not free of obstruction, such as (but not limited to) uneven concrete and uneven plywood laid on the ground.  Furthermore, the scissor lift was parked and set up on an uneven ground surface (not within three (3) degrees of level) and the scissor lifts working platform was fully extended upward to reach the elevated work area where the employees were installing curtain wall head clips.          On or about October 16, 2019, the above scissor lift used to gain access to the elevated work area to perform the above task tipped over resulting in the employees falling approximately 19 feet to the ground and sustaining serious injuries.
