29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  On or about October 16, 2019, a chute was not used to remove roofing debris from the roof of a building with an eve to grade greater than 20 feet, exposing employees to struck-by hazards associated with dropping material without a chute.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  On or about October 16, 2019, employees performing roofing operations were not protected against electrical shock/ electrocution while working around an energized 400 amp commercial service drop, exposing employees to the hazards associated with electrical shock/ electrocution.
