29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function. MV Islander, main deck, stern: Employees are required to don 3M 1/2 (MD. 600) and full face (MD. 3M P-100), tight fitting respirators to protect against the health hazards associated with organic vapor generated when painting or using paint thinner have facial hair preventing a proper seal.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v). MV Islander, main deck, stern: A "Do it Household" spray bottle used for cleaning purposes was not labeled with the identity of the chemical nor the appropriate hazard warnings. Employees are potentially exposed to chemical hazards.

29 CFR 1915.73(f): Where open sections of bilges were in a walkway, at least 2 10-inch planks were not placed side by side across the opening to provide a safe walking surface. MV Islander, engine room: A section of floor grating (23"x4"x 8") was removed from a walkway and not guarded to prevent slip, trip or fall hazards. The condition was abated at the time of the inspection.
